The following is required for all telephone consultations:
- A current medical record dated with in the last 12 months old- your physical exam, including progress notes, and/or chart notes from your PCP- Primary Care Physician (local healthcare provider). We do not accept hospital discharge papers or insurance papers.
PLEASE NOTE: If you do not already have copies of these records, you may request that your PCP (Primary Care Provider) forward them to us directly. Print out our “Records Release Form”, sign it, forward to your PCP who will then send all records directly to us at Accurate MD, 409 N. Pacific Coast Highway, #483, Redondo Beach, CA, 90277 or fax to 1-866-350-8090.
***Failure to have all of this information in your patient file WILL result in delayed Initial consultations, Follow-up consultations and refills***

| Scheduling Your Doctor Consultation |
|

After you’ve made your payment, you will be notified via email to schedule your consultation. At that time, you will be prompted to login to Accurate MD, select “Patient Menu” in the task bar and then select “Request Consult” from the drop down menu. |

The Physician will contact you at the number you have listed in your registration. Please make sure to keep this information current. You may update your account information by selecting the "Patient Menu". |

We strongly recommend that you do not miss your assigned consultation time. In accordance with our Terms we will charge patients for all missed consultations. |
|
|
Our physician will be the ultimate decision maker on whether or not to treat your condition. |
|
|
Please prepare any questions you may have for the doctor prior to your consultation. The doctor will cover questions you have regarding your condition, use of prescription medication if written and any treatment options available.

Ordering Your Pharmacy Medication Refills |
|
The doctor writes a prescription with a 30 day supply. To receive your refills in a timely manner patients are able to request refill(s) on the 25th day (count from the day after the date on the bottle). You must contact the pharmacy directly to request your prescription refills. |

Upon receipt of your prescription be sure to read all instructions that pertain to your medication.
Any questions regarding your refill(s) need to be directed to the Pharmacy. |

Changing Your Doctor Consultation Date/Time |
|
If you would like to change your consultation date/time please email Patient Support and we will attempt to move you to your desired date/time. If you need a consultation today and there are only times available tomorrow please complete the process and pick the earliest available schedule time tomorrow that would work for you. You must complete the process and be on the schedule before we can move your date/time.
